Nebraska Transitions Disabled Waiver Service from League of Human Dignity to DHHS

LINCOLN, Neb. – The Nebraska Department of Health and Human Services (DHHS) is transitioning Aged and Disabled (AD) Waiver service coordination from the League of Human Dignity (LHD) to the department as part of an ongoing effort to deliver high‑quality, efficient services to Nebraskans.

The state has a long history of providing service coordination to AD Waiver participants. All waivers have quality assurance measures identified and approved by the Centers for Medicare and Medicaid Services, with established reporting requirements. The quality assurances will not change.

DHHS currently employs experienced service coordinators and has the internal infrastructure needed to support this change during the transition. DHHS is committed to maintaining continuity of care and ensuring all Nebraskans receiving AD or Traumatic Brain Injury (TBI) Waiver services experience little disruption.

The service coordination transition for participants on the AD Waiver will continue until March 31, 2026. Beginning April 1, 2026, all service coordination duties held by LHD will be transitioned over to DHHS.

DHHS is finalizing the hiring process of additional staff to support the increased service coordination caseload and is actively hiring high-performing LHD service coordinators, allowing many participants to retain their current coordinators. 

For participants assigned to a new service coordinator, DHHS will provide the new service coordinator's name, full contact information and the direct supervisor’s information to ensure clear communication. All affected waiver participants will be contacted before April 10 by DHHS.
