CHEYENNE, Wyo. (AP) — U.S. accident investigators say brake failure probably caused a freight train collision that killed two crew members in southeastern Wyoming in 2018.
The National Transportation Safety Board says in a report an air flow restriction probably caused an air brake system to fail as a train consisting of three leading locomotives and 105 cars descended a 13-mile grade.
The train hit the rear of another train, killing an engineer and conductor.
Three locomotives and 65 train cars derailed.
The NTSB says Union Pacific failed to maintain rail cars in accordance with regulations.
Union Pacific says it has implemented its NTSB recommendations.
